Rapamycin and the feline kidney: the study everyone hopes exists, and what actually happened
Kidney disease is the thing most likely to take an old cat, so it is where hope runs hardest. Here is the honest state of the evidence on mTOR inhibitors and the kidney — including the human trials that disappointed.

If you own an ageing cat, chronic kidney disease is the diagnosis you are quietly dreading. It is one of the most common causes of death in older cats, it is progressive, and by the time the classic signs appear — drinking more, weight loss, a dulling coat — a great deal of kidney function is already gone.
So it is completely understandable that owners searching for "rapamycin for cats" are often, underneath, asking a more specific and more desperate question: could this help my cat's kidneys?
You deserve a straight answer, and this site exists to give straight answers even when they disappoint. Here it is.
The honest headline
Rapamycin is not an established treatment for feline chronic kidney disease. There is no controlled trial showing it preserves kidney function or extends life in cats with ordinary CKD. Anyone implying otherwise is selling you something.
That is the conclusion. The rest of this article is why the question is nonetheless a reasonable one to ask — and why the nearest real evidence is a cautionary tale rather than a green light.
Why the kidney and mTOR are genuinely linked
This is not a case of a hopeful marketer bolting a trendy molecule onto a scary disease. There is real biology here.
mTOR drives cell growth and proliferation, and two of the ways kidneys fail involve exactly that: fibrosis (scar tissue crowding out working tissue) and, in one specific disease, the growth of fluid-filled cysts. mTOR signalling is implicated in both. That is why researchers took mTOR inhibitors into kidney disease at all. The rationale is sound.
The disease where this was tested hardest is polycystic kidney disease (PKD) — and PKD matters to cat owners, because it is inherited in Persian and related breeds through a known mutation. Cysts grow, the kidney enlarges, function declines.
What the human trials actually found
Here is the part the hopeful version leaves out.
mTOR inhibitors — sirolimus (rapamycin) and its relative everolimus — were taken into randomized human trials in autosomal dominant PKD, on exactly the reasoning above. The results were largely disappointing. In the major trials, the drugs did not meaningfully preserve kidney function, and the effect on cyst growth was modest at best; meanwhile side effects were common (Walz et al., NEJM, 2010; Serra et al., NEJM, 2010).
That is the crucial lesson, and it is a lesson about this whole field: a beautiful mechanism is not a result. The theory said mTOR inhibition should help polycystic kidneys. The trials were run. The theory largely did not survive contact with patients.
And note what this means for cats specifically. Even the disease with the strongest mechanistic case for rapamycin — inherited PKD, which affects Persians — is the one where the human evidence came back discouraging. For ordinary feline CKD, which is not primarily a cystic disease, there is not even that much to go on.
What about the RAPACAT trial?
Cats do have one controlled rapamycin trial: RAPACAT. It is worth being precise about what it was, because its existence sometimes gets stretched.
RAPACAT tested delayed-release rapamycin in cats with subclinical hypertrophic cardiomyopathy — a heart condition, not a kidney one — and it was a safety and tolerability study. It found the drug was tolerated at the doses studied. It was not about kidneys, it was not about lifespan, and it does not transfer to CKD. What genuinely helps a cat with kidney disease
This is where the honest article earns its keep, because the interventions with real evidence are not exotic:
- Catch it early. A blood test called SDMA can flag declining kidney function earlier than the older creatinine test, which means starting management before more is lost. This is the single most useful thing, and it is why a mature cat — from about seven — should have bloodwork before anything looks wrong.
- Therapeutic renal diets are one of the few interventions shown to slow progression and improve survival in cats already diagnosed. Not glamorous. Actually evidenced.
- Managing the downstream problems — blood pressure, phosphate, hydration, protein loss — is what a good CKD plan is built from.
- Staging and monitoring through your veterinarian, using the international IRIS framework, so decisions track the disease rather than guesswork.
None of that involves rapamycin. All of it has better evidence than rapamycin does for this disease.
The bottom line
The question "could rapamycin help my cat's kidneys?" is reasonable, because the biology really does connect. But the honest answer today is: the mechanism is real, the human trials in the best-case disease disappointed, and there is no controlled evidence in feline CKD. Hope is not the same as evidence, and we will not blur the two — that blurring is the entire thing this site exists to refuse.
